I, the parent/guardian of this child, authorize any official agent of Northlake Bible Church (NBC) to use his best judgment if I cannot be reached during an emergency involving illness or injury. I give my permission to NBC to transport my child to suitable medical personnel and for aid to be rendered as necessary. I understand that my personal insurance is my primary coverage. I do hereby release NBC, its officers, employees, agents, and members of the Board of Elders from all claims and causes of action by reason of any injury which may be sustained as a result of church activities. I understand that NBC photographers may photograph my child during VBS activities this week for limited use and viewing among the church and VBS participants, but that no personal information about my child will be made public. I have read and agreed to the above statements.
